The Monarchs Inner City Youth program is committed to developing young athletes through sportsmanship, teamwork, and respect. This Code of Conduct ensures that every participant, parent, and coach contributes to a positive, safe, and supportive environment.

Media Release Consent

I, the undersigned, hereby grant Monarchs Inner City Youth permission to record, photograph, and/or video my child in connection with official team activities, practices, tournaments, events, or community functions.

I understand that these photographs and recordings may be used for promotional, educational, or informational purposes including, but not limited to, social media, websites, brochures, newsletters, and other official publications of the organization.

I waive any right to inspect or approve the finished product or any written copy that may accompany the images or videos. I release Monarchs Inner City Youth, its representatives, volunteers, and affiliates from any claims, liabilities, or damages that may arise from the use of such materials.

Consent Options

I give permission for my child to be photographed, videotaped, and included in Monarchs Inner City Youth media.

I do NOT give permission for my child to be photographed or recorded.
